Medical Forms and Summer Camp Information


In order for your scout to participate in activities such as camping and other day programs, we must have the appropriate medical forms. These forms ensure your son’s safety and well being, and give us proper information and authority should medical intervention be needed.

We will keep these forms on file for your scout, and will bring them to all troop activities and campouts.

Should your son’s medical or contact information change, you are responsible for updating the Scoutmaster and making sure a new medical form has been submitted to the Troop.

Prescription and non-prescription medications, such as allergy medicine or pain relievers, should be given to the Scoutmaster in their original containers. The Scoutmaster will be responsible for making sure all medications are given at the appropriate times indicated on the container.

Health Form Parts A and C
- We need this form for any activity your scout might do with the troop. this form is filled out by the parent and does not require a doctor’s signature. This form is needed for all troop weekend camping and activities.

Summer Camp Forms

Health Form Part B
- Must be signed by a doctor, and mandatory for all scouts and adults to attend summer camp.
Individual Medication Form
- Must be signed by a doctor as well. This one says they can give your child over-the-counter medications if necessary.
Prescription Medications Form
- Must ALSO be signed by a doctor. This is for any scout bringing prescription medications to camp.
